Workplace Learning — Performance Improvement CME: “What’s in a Name?”

I see a lot of things going on in the PI (CME) arena now. Some of the Performance Improvement Modules (PIMs) that are being offered are really good and are getting a lot of attention. Part of the popularity is in part due to the Maintenance of Certification (MOC) requirements. The sizzle in those steaks is loud and clear. But PIMs are not the only form of effective PI CME.

I am hearing some, but not much, about PI CME in the work setting. These efforts have nothing to do with elaborate computer guided PIMs. These efforts are driven by physicians in a practice setting looking at some part of that practice to see if they are meeting evidence based expectations. If they are not meeting national standards or peer expectations they look at the work they are doing to find out why, change that work and re-assess the outcomes they are achieving. In some instances formal learning may be a part of the solution. Hooray for CME. In some instances formal learning is not a required intervention. Hooray for Performance Improvement. Is this PI CME? You bet.

Perhaps we should call this form of “performance improvement” “Workplace Learning”. That is what it is. Physicians and care providers generating knowledge about their care in the context of care – where they work. Some of that knowledge will be unique to that setting. “Place matters”. Behavior is a function of the environment in which it occurs.

All patient care is contextual – it happens someplace. That place has an impact on the care provided. Improving care requires that the context in which it occurs be given consideration. Knowledge created through workplace learning will not be knowledge for knowledge sake. It will be knowledge that is put into practice to improve the care provided in that context.

“Workplace Learning”. I kind of like the sound of that. What do you think?
